    Item Description:

    The Global Fund to Fight AIDS, Tuberculosis and Malaria (the “Global Fund”) invites all potential suppliers (the “Bidders”) to submit proposals for the above-captioned Request for Proposals (RFP).

    This RFP is comprised of the following sections and attachments:

    • Section 1: Terms of Reference
    • Section 2: Proposal Requirements and Evaluation
    • Section 3: Proposal Submission
    • Section 4: Legal Matters
    • Annex 1: Certificate of Conformance
    • Annex 2: The Global Fund Terms and Conditions of Purchase of Services (“Terms and Conditions”)
    • Annex 3: Rate Card Template

    The Global Fund 2023-2028 Strategy, entitled “Fighting Pandemics and Building a Healthier and More Equitable World” includes four mutually reinforcing contributory objectives which underpin the Global Fund’s Goal to end AIDS, TB and Malaria:

    1. Maximizing people-centered systems for health to deliver impact, resilience and sustainability
    2. Maximizing the engagement and leadership of most affected communities to leave no one behind
    3. Maximizing health equity, gender equality and human rights
    4. Mobilizing increased resources

    These objectives are complemented by an additional evolving objective: contribute to pandemic preparedness and response. To maximize impact of these investment areas at country level, the Global Fund is committed to maximizing impact of investments and supporting countries to eliminate implementation bottlenecks. Supporting efficient and reliable health laboratory services traverses all five of these contributory and evolving objectives, with resilient, integrated laboratory systems forming the foundation of health systems in all countries.

    Laboratory services are an essential component of any resilient health system and are central to achieving the core mission of the Global Fund. Laboratory diagnostic and monitoring capacity is critical towards achieving the global targets for Human Immuno-Deficiency Virus (HIV), Tuberculosis (TB) and Malaria control. Global Fund is investing significantly in laboratory services -approximately 11.5% of total grants in the last funding cycle with a significant proportion of this investment dedicated to laboratory equipment.

    However, much of this investment in laboratory equipment has not been optimal with low equipment utilization rates, high cost per test, limited or no maintenance of equipment, limited access to and poor quality of testing. Improving utilization is crucial to a cost-effective system as the cost per test decreases with higher testing volume. Additionally, with the introduction of newer innovations like point of care technology, there is a need to understand how best to maximize and optimize their use within the entire laboratory network. In order to address these challenges, multiple countries have sought to address the root causes of many of these barriers from a broader, systems-wide approach, known as laboratory network optimization.

    Gaps in laboratory systems and inadequate diagnostic capacities present some of the most intractable and complex barriers to achieving disease-specific targets for HTM. Inefficiencies in laboratory operations and systems are multi-factorial, arising from weaknesses at all levels of governance/leadership, management, infrastructure and operations. To date, there has been limited involvement of national laboratory leadership in grant development and the life-cycle of global fund country allocations, which may lead to poor alignment between interventions and NLSPs and exacerbate the already limited visibility and limited domestic investments for lab systems and services.

    There is a need to drive coordinated approaches towards laboratory system strengthening in Global Fund-supported countries, leveraging best-in class digital and connectivity solutions, developing advanced human resources capabilities in Lab Directorates and other lab coordination bodies, establishing and embedding best practices, modernizing equipment management and deployment, and enabling sustainable country-driven development of integrated lab systems.

    Needs for technical assistance (TA)

    Needs for technical assistance are identified in collaboration with key stakeholders in the Ministry of Health, Lab Directorate, Lab Technical Working Group, national disease programs and other relevant stakeholders. Activities prioritized for assistance respond both directly and indirectly to disease-specific response under HIV, TB, Malaria, SARS-CoV-2, as well as newly eligible activities in support of pandemic preparedness.

    Three key principles guiding TA procured to support countries are:

    • Country demand-driven models: Adequate buy-in must be secured prior to deploying TA to a country; to ensure acceptance, sustainability and embedding of results, TA must be delivered with cooperation and support of key sponsors in the Ministry of Health and / or Lab Directorate.
    • Promotion of integrated and innovative approaches: TA for Laboratory Systems support must be innovative, cross-cutting, supporting horizontal improvement of the laboratory system without isolating support between disease programs. TA activities must address system improvements rather than disease-specific outcomes.
    • Country participation and sustainability: TA must be undertaken on-site, in-country to enable adequate knowledge transfer and development of human resources capabilities.

    Prioritizing TA

    Some of the root causes of limited coverage and access to quality assured diagnostics and disease monitoring services relates to weak laboratory governance structures across national networks, delays in adaptation and implementation of global diagnostic guidelines, and limited development and implementation of integrated National Laboratory Strategic Plans (NLSPs). There is a critical need to coordinate partner engagement for TA support to reinforce the convening power of Lab Directorates, improve good lab system governance , develop laboratory mentoring programs, and to encourage effective uptake of proven innovations.

    Priority regions

    Disease response in LMICs has been hindered by insufficiently supported lab systems, and under-investment in laboratory technical expertise, as evidenced by disruption to laboratory services during the COVID-19 pandemic. Strain on existing laboratory systems is reported to be more pronounced in the regions of Western and Central Africa.

    There is an urgent need for TA providers capable of deploying high quality technical assistance in Francophone and Lusophone countries in West and Central Africa.
